Specialized Transportation Trip Application Form
Submit the Trip Funding Application Form below to indicate what trips you would like to receive funding for. You must arrange your own transportation, and you must keep your receipts and provide them to District staff.
You can apply for funding for trips in advance of when you plan to take them, so that you do not have to pay out of pocket. You will need to provide a reasonable estimate of the cost of the trip. Please submit this form a minimum of THREE WEEKS before your planned trip to ensure we have enough time to process the application and get the money to you.
You can apply for funding for several upcoming trips at once if you know you will have to take them in the near future, but funding will only be provided for one trip at a time. You will have to submit your receipt(s) as proof of travel for your first trip before funding will be provided for your next trip.
You can also submit this form to apply to receive money to reimburse you for a trip you have already taken. You must provide a receipt for your trip in order to receive reimbursement.
Once you are enrolled in the STP program, you can apply for additional trips when you know about them. Just remember that each individual can only receive a total amount of $2,000, so once you reach that limit, you will not be able to access funding for any more trips. As mentioned above, you will need provide a receipt for any past trips you've taken before funding for additional trips are approved.
