Specialized Transportation Fund Application Form

The Specialized Transportation Fund (STF) can help people who need financial support for transportation and who do not have access to public transportation. To apply for STF, please fill out the following form.

If you need help completing this application, please send an email to specializedtransportationfund@muskoka.on.ca  or (705) 645-6764 ext. 4417.

Please note:  This application is valid for a one calendar year.

Please read the following before answering the next question.

Total household income includes the total combined income of all your household members after all income taxes have been paid. Income includes money received from employment wages, employment insurance, old age security, the Canada Pension Plan, investment income, a private pension, Ontario Works or the Ontario Disability Support Program. Please add up the total income of all members of your household from all sources, but not including the amounts everyone had to pay for income tax, in order to answer this question. If you live alone, your income minus your taxes will be the total household income. If you live with a partner or anyone else who earns income, please include their after tax incomes when you answer this question.

Please refer to your income Tax Assessment – Line 15000 for your total yearly income.  You may be asked to submit this document.
