Welcome families! We are happy that you have chosen to have your child registered with the Muskoka Home Child Care Agency. Your child’s registration is an essential part of the intake process and the first step in securing a space. Consider the following:

This form will take you approximately 30 minutes to complete.

You will require the below documents while filling out this form. Please download and complete them before continuing, so you can upload them when you are prompted.

• Up-to-date Imunization Record OR a Statement of Medical Exemption form OR Statement of Conscious or Religious Belief form for non-school aged children
• Simcoe Muskoka District Health Unit Immunization History Form for non-school aged children
• Custody documentation, if applicable
• Family Contract
• Pre-authorized Debit Form
• A void cheque or banking account summary from your online bank

If it is determined during the enrollment process that your child requires an Individual Medical Plan, an Individual Support Plan or an Anaphylaxis Package to be completed, a delay in the enrollment process may occur. It is important to inform the Agency as soon as possible if you are aware that an individual plan is required.

Registration packages must be completed at least 2 weeks before your child’s start date. 

At any time, if information changes on this form, it is the responsibility of the parent/guardian to inform the Home Visitor assigned to your child care Provider.

The information you provide in this form is collected in a safe and secure manner and collected in accordance with s.29(2) of the Municipal Freedom of Information and Protection of Privacy Act and pursuant to section 71 of the Child Care Early Years Act, 2014 and will be used for the administration of the Directly Operated Muskoka Home Child Care Agency.
